How To Register A Death in Ireland:

When a death occurs, the death must be registered by a relative or civil partner. When someone passes away either in a Hospital/Nursing Home/Institution OR at Home, a Death Notification Form will be issued to the next of kin by a Doctor from either institution or the deceased’s GP.

Part 1 of the Death Notification Form will be completed by a Doctor and will include the deceased’s name, address, and cause of death along with the stating the date of the last time the medical practitioner examined the deceased prior to their death.

The relative (by blood OR by marriage) or civil partner should complete Part 2 of the form which gives further information on the personal details of the deceased.

The Applicant should sign the Part 2 of the form and bring it to your local Civil Registration Office to register the Death. They should also bring CURRENT Picture ID to identify themselves to the Register. A copy of this will be kept on file as proof of who registered the death.

A death can be registered in any part of the Republic of Ireland regardless of where the death occurred in the Republic of Ireland.

The death must be registered within 3 months of the death occurring.

If there is no relative or civil partner available OR capable of registering the death, either at the point of the Death Notification Form been issued by the medical practitioner, OR after the 3 month period has passed, a death can registered by the following people:

  • A person present at the death
  • Any other person who has knowledge of the required particulars
  • If the death occurred in a building used as a dwelling or a part of a building so used, any person who was in the building or part at the time of the death
  • If the death occurred in a hospital or other institution or in a building or a part of a building occupied by any other organisation or enterprise, the Chief Officer of the institution, organisation or enterprise (by whatever name called) or a person authorised by the Chief Officer to perform his or her functions.
  • A person who found the body of the person concerned.
  • A person who took charge of that body.
  • A person who procured the disposal of that body
  • Any other person who has knowledge of the death

Once the Death has been registered by the Registrar, Death Certificates at the cost of €20.00 each can be purchased.
